Questions and Answers for Fiscal Year 2016

1. Does a potential bidder need to show proof that they are already providing services in an area they are considering bidding?

Answer: A bidder is required per the RFP to already have the address (office location) and required staff at the time the RFP is submitted. In other words you cannot wait until award to rent office space or hire staff to provide service.

2. Does the prospective bidder have to have already been providing substance abuse/mental health services from that address, or if they secure a building before submitting the proposal is that allowed?

Answer: At the time of submission, the lowest priced technically acceptable bid will be contacted for a site visit. At the time of the site visit I must be able to see the inside of the building and check for code compliance etc. If the office is not completely furnished or is not currently seeing patients that would be ok as long as it would be fully functional for services referred on Oct. 1.

3. What is the requirement for geographical location distance. For example, would an office in Woodland be sufficient to serve Fairfield people or does the office have to be in the County of the contract?

Answer: In each of the RFP's there is a geographical area noted on page B-1. It must be in that noted location, for instance if it says Sacramento it must have a Sacramento address. If it says Fairfield/Vallejo the services must be performed in one of those locations.

4. Are there specific instructions if you use subcontractors?

Answer: The RFP discusses the use of subcontractors in several areas of the RFP. If you plan on using subcontractors make sure they meet all criteria before submitting the RFP. Also, the following language is found at L-2 of the RFP:

Subcontracting

For service items that the offeror will be subcontracting, the offeror shall insert the letter "S" following the price inserted in the Unit Price column.

Services referred to another vendor shall be considered subcontracting and shall require the "S" designation.

5. IF a therapist has All documents EXCEPT the signed compliance- Do I need to delete from "Staff Qualifications" or submit RFP without the signed compliance and then provide a signed form upon RFP acceptance?

Answer: Preparation of Certification of Compliance Statement (Attachment A)

Each offeror shall prepare and submit as part of its offer a CERTIFICATION OF COMPLIANCE STATEMENT in which the offeror certifies that it will provide the mandatory requirements stated in Sections C, E, F and G and comply with terms and conditions of the RFP. If the offeror is proposing subcontractor(s) to perform any services, the offeror shall identify the proposed subcontractor(s) and submit separate certification statements from each subcontractor that certifies that they will provide services in compliance with the requirements of the RFP.

Fiscal Year 2016

1. A session is 30 minutes, although our services are usually referred at the rate of one hour or one and half hours, our estimated monthly quantities are given in 30 minute increments.

2. Project code 2022-manualized CBT programs require the vendor to provide the course materials so the price quoted should account for this cost.

3. The attachment J.1 is not submitted as part of the offeror's proposal. It is used post-award.
